I like your translation. I've translated this song as well for our praise and worship team and it's similar. When I translate I try to stay as true to the meaning as possible but still make it musical, like you've done. But I think I might take some of your words and adjust my version. In case I might help you to do the same thing, here's my translation. Maybe the translation of "altogether worthy" for example - as "tan sumamente digo". It fits well and is a literal translation.
