The break-up of a relationship is a topic that has been covered countless times by countless artists. So there is an unstated expectation to bring something new or unique to the topic in the format, phrasing or context of your poem. While it may be an exacting description of your feelings or experience, I did not find a fresh face in this poem. I do like your format and the conflict that is presented between holding on to joys of the past and finding the strength to move on.
