    My question is about the structure of "what if"...I'm unsure about if I have to express it with a verb or not, as in German for example we say "was wäre wenn"

    I noticed from the examples you posted that yes, there's verb and in the past tense.

    You can also use ماذا إن instead of ماذا إذا
    Even in your last example you posted
    if he is sick he shouldn't come at all
    إن كان مريضًا، فليس عليه الحضور

    so the translation will be also:
    وماذا إن حلمت وأنت نائم.... إلخ

    I told that you use the past tense of the verb, But if you want to use it with negative form then, you should use the present tense of the verb, like

    (what) If you didn't sleep
    (ماذا) إن لم تنم
    (what) if you didn't dream within your sleep?
    (ماذا) إن لم تحلم خلال نومك؟
    (what) if you didn't hold the flower within your hand?
    (ماذا) إن لم تكن تحمل الأزهار بيدك
    لا يَشْكُرُ الله مَنْ لا يَشْكُرُ النَّاس

    is gramatically incorrect.
    Your English sentence says : If I am dreaming, I do not want to wake up
    so I wake up: أستيقظ
    and the translation of the Arabic sentence you posted will be: if I'm dreaming and I want you to wake up.

    that was one.

    and second it's not suitable to use wa here, because wa means and, unless there were another thing you want to say.
    It'd be more suitable to say fa which means so.

    you should say
    أن أستيقظ:which means I wake
    and not
    أن تستيقظ : which means you wake
    لا يَشْكُرُ الله مَنْ لا يَشْكُرُ النَّاس
