It means: Do I know you?
Or: Do we know each other?

But it's like: Do we know each other?!? (someone is surprised you approached him/her because he doesn't know you)

and the answer depends on.. the answer lol

if i wanna answer "i think we are family" how do i say that?

Mislim da smo obitelj.
or
Mislim da smo rođaci (that's kind of like: I think we are cousins/relatives)

or
Mislim da smo u nekom srodstvu
