I can't find the name of this Christian, acoustic guitar song

Hey,
I am looking for the name and/or artist of this Christian, acoustic guitar song...the singer is a man and these are some lyrics I know.
These are the first two lines: "Take my heart and let me feel, take my faith and make it real"
And later on it goes like: "Don't let me go, hold me close to where you are."

If anyone has any idea what this song is, I would love to know!! Thanks!

I'm pretty sure that's Third Day. It's from their CD "Offerings" and is the extra song hidden at the end after "Love Song."
