Well, technically the formal is using Usted (always abbreviated "Ud.")...
    "¿Cómo está (Ud.)?"

You can say "¿Qué hay de nuevo?" for "What's new?" also!

so formal way to speak with "usted" ... I only use it with my parents, boss or teacher.. senior people...

but formal and with respect you can also use: como estas? ........

informal ways to ask how are you? ... are

    que tal?
    que onda?
    que hay de nuevo?
    que cuentas?
and many more with slangs or even bad words.. mexicans are fluenty on bad words hahaha

Right, I thought Viva meant more like "strict" formal... I wouldn't even use Ud. with relatives, except an old 90-year-old, simply out of respect (and that generation was brought up more formally than us modern-day youngsters )
