Well, technically the formal is using Usted (always abbreviated "Ud.")...
"¿Cómo está (Ud.)?"

Right, I thought Viva meant more like "strict" formal... I wouldn't even use Ud. with relatives, except an old 90-year-old, simply out of respect (and that generation was brought up more formally than us modern-day youngsters
