Just a few small corrections:

"di notte esce la forza di non dirmi che hai bisogno di me": in this case "esce" means "to find out" , "at night you find out the courage to don't tell me that you need me"

e fa male quando dici che stai male..... :"Stare male" means to "feel ill/bad"

e fa male col dolore che t’assale...: it can be "and it hurts when the pain that strikes you and I'm not with you"
